HELP!! I just cant get how the discharge UB04 should look. Lets say the patient was in a SNF 07/01/08 thru 07/15/08 with 07/15/08 being the discharge date. What would I put in these boxes? I know insurances dont cover the discharge date but you have to have it somewhere on the claim right?
Box 6-state covers period-does this include 07/15?
Box 35-occurance span-again same question
Box 39-since there is no charge for the discharge date-what occurence code is used if any?
Box 45-I know this only shows the start date of the claim and then has units which would be 14.
I will get this if it kills me!! :'(

Hi Chris,

I will try to help!

For Box 6 I would say enter the admit date thru the discharge date.  Even though the discharge date isn't covered, if you don't put it then they will think the day before the discharge is the discharge date.

For Box 35 I would do the same thing.

Box 39 is for value codes, and I don't see one that applies to what you have described.  It is not a required field, but if you see one that applies you should use it. 

Box 45 - you got it.

Now that didn't seem so bad when you take it one box at a time.

My only concern now is with box 35 as I use occurence code 70 which is covered days and the discharge is not covered. But i think I have been doing it that way anyway. I get a little confused, but I guess thats the name of the game Not billing specialist CONFUSION SPECIALIST!

Many people have no idea exactly what is involved in billing.  It is definitely much more involved than people who don't do billing realize!  :)
