If you are a billing service company and offer pre-authorization and insurance verification, how do you guys handle that. Do you customize some kind of forms for the providers when they need to verify eligibility or what. I'm not sure how you would do it since you're not at the office. Do you give the providers the form and they send it to you when they need this service.

We have done this and we just had a simple form that could be filled out during a phone call.  Name, address, DOB, insurance company, ID#, Group #, and phone number for insurance.  The provider would fax the form over to us, we would call.  There was a place for notes on the bottom of the form and we would fax it back with verification info.
