Why are you trying to convert it to a PDF?  You mean a blank CMS?

Are you trying to convert from word Doc, if yes just google it - you have quiet a lot of websites that would allow you to convert a word to pdf file.

Doesn't the course provide a CMS form?

The course does provide a CMS form.  Did you try emailing the PMBA. You can also fax them as well.
