Has anyone billed these psych codes?
« on: June 17, 2014, 10:03:23 AM »
One of our clinic locations serves a specific immigrant population with native language mental health services. The patients typically need extra help in navigating the larger system so our therapists spend a lot of time communicating with/going over records and letters from other medical providers, caseworkers, and the state for these people. I'm talking way above and beyond what would be considered "included" in a normal psychotherapy/psychiatry appointment. A patient might have 45 minutes of psychotherapy, which is billed, but then the providers spend an hour or more of additional time on these other coordination activities. They are asking me if there are other CPT's they can bill for these services. I have found some that exist, but of course actually getting them paid is a different matter. My first concern is they'll just be bundled with the psychotherapy codes. Has anyone ever tried to bill any of these along with psychotherapy codes or diagnostic evaluations?

90885 - Psychiatric evaluation of hospital records, other psychiatric reports, psychometric and/or projective tests, and other accumulated data for medical diagnostic purposes.

90887 - Interpretation or explanation of results of psychiatric, other medical examinations and procedures, or other accumulated data to family or other responsible persons, or advising them how to assist patient.

90882 - Environmental intervention for medical management purposes on a psychiatric patient's behalf with agencies, employers, or institutions.

90889 - Preparation of report of patient's psychiatric status, history, treatment, or progress (other than for legal or consultative purposes) for other individuals, agencies, or insurance carriers.

No, those codes would not be appropriate, you want to look at E/M codes 99201-99205  and 99211-99215 as these codes will include "coordination of care".
