Do you want to know how much to bill or how much is reimbursed??? The provider should already have his own fee schedule.

Hi...we price our drugs based on highest contracted reimbursement rates/versus invoice for the drug in question. I have also seen where the drugs were priced anywhere from 150% to 300% over the Medicare Fee Schedule, which can be found at:
