Also a recommendation for an HRA tool/questionarre?

Lastly, can you bill a AWV + ACP + Depression Screen on same visit?

Are you looking for a superbill?

I'm not sure what an ACP visit is.

You can bill for an AWV & depression screen on the same visit.  You may need to append  the 25 modifier if warranted.
