What to Charge ? New Practice

Would like to know what to charge a provider that see less than 10 patients  per month? Brand new office and provider don’t want to be over charged. (Speciality InternalMedicine). Provider has about 50 active established patients and still trying to establish. Any suggestions would be great...

Wow, they are only seeing 10 patients per month?  I know they are brand new, but that is very low.  It's hard to give you a specific amount without knowing more.  What area of the country are they in?  Rural or city? PCP? Are they doing any labs directly (UA, Strep)? 

So on a broad picture I would say that you should probably do a sliding scale pricing model to allow for the growth. 

Maybe something like:

0-10 patients seen per month  =  $50.00
11-15 patients seen per month  =  $75.00
16-20 patients seen per month  =  $100.00

etc.

Please remember that this is a very rough estimate as I have little details to go by.  I just wanted to give you an idea.

Charlotte, North Carolina She does labs in house. She said that she was using Advanced MD when she first open and they were charging flat fee of $1,500.00 Per month. She also mentioned when Advanced MD was billing she was only getting $100-$200 Reimbursement. So with that being said she felt like she was being overcharged.

There isn't an emoji available for the expression on my face.  She was being charged $1500 a month to bill 10 claims?  That seems excessive. 

If possible I would try to get additional information.  Can she provide you with a couple of EOBs so that you can see what is being billed and what is being reimbursed?  I would be curious to see what she is billing vs what she is being paid, as well as if charges are being denied and if so, for what. 

With the little bit of information given it appears she is being overcharged.

Could they have been using AdvancedMD software along with their billing services? Yes obviously there is a big cost that a small practice wouldn' t need..but before saying that there are a few things to consider. If the practice is brand new could it be that the practice will grow INTo the services and fees? If they cut out AdvancedMD and their billing services this leaves the practice with another expense with software/support/clearinghouse and then your fees OR if they use your system you will want to make sure you are compensated for the setup (conversion/manual conversion) into your system. So maybe they won't pay $1500 a month to start there should be a startup fee and then the pricing should accomodate not only the volume today but the goals of the practice, staff growth, patient potential volume, credentialing etc. Now if this practice is somewhat already grown as much as they want or have the goal to grow then IMHO it would be better to consult with them to setting up in-house with a less costly but long term solution.

When I take on a new client I do my very best not to compare costs of another billing company. I don't get involved in what they were paying or trying to beat their price, etc.

No offense, but 10 patients a month is peanuts, granted they are a new practice and that might make sense. I would agree with Michele, a sliding scale sounds like the way to go here. $1,500 is way too high for 10 patients, something is wrong there.

I charge 7% of what I collect. Plus whatever the doctor chooses to write off for a particular patient.  ;)
