What code can be corrected from this...

Code POS TOS M1 M2 Units Error
64635 11 2 LT X2 1 CO-50 M25 N115
64636 11 2 LT X2 2 CO 234 N390
99070 11 9 1 CO 97 N19
To correct codes for different levels.

It is not possible to answer your question simply based on the information provided. But I would suggest that you look for an NCD/LCD for codes 64635 and 64636.

Error shows : This service/report cannot be billed separately.Procedure code incidental to primary procedure. Based on LCD dx code M47.817 is active. So which code needs to be corrected.

Check the NCCI/CCI edits for the codes, and it will tell you which code needs to be corrected.

I find this offensive. Clearly people have tried to help you but we've also indicated that we don't have all of the information to be able to resolve the issue completely. When did it become our jobs to help you do yours? This is a forum where billers can come to ask questions, get help, search previous threads for research, or help others. It is free. You come on with a question and give minimal information and then get upset because no one gave you the answer you are looking for.
Did you look at the NCCI/CCI edits that Kristen suggested? The N115 code usually references an LCD. Did you read that?
