Need to find "Bill Type"

Where can I find information on “Bill Type”, without having to buy a huge book?  I have insurance co’s rejecting my type of bill---that I have been using for years??

We ran into the same problem a few years ago and had to buy the “huge book” (over $300).  Not only is it expensive, but hard to find the information you need for anything other than a hospital. 

We wrote a book on completing the UB04 forms and have a whole section on the “bill type” with examples for different situations.  Here’s more information on it –
