Medicare Authorization Form

Hello,
We're hopefully going to be filing for Medicare soon.  It's been years since I've done this, but when I worked the front in a physician practice, I remember having to make sure each Medicare patient signed a form that we kept in their chart authorizing us to file their Medicare.
Could you tell me if that is still required, and if it is, where to find the forms.
