Fee schedule: Unit price VS State Max

Hello All,

I am needing clarification on how to charge using the fee schedule. What is the difference between the unit price fee and state max fee and how can I make sure I am using the correct fee? Another question, some labs have no unit price but has a state max price, am I to use the state max?
