CGM Billing and Criteria

Hello,

Could you please help to know about below criteria for Continuous Glucose Monitor (Diabetic) .
1. Can we bill claims to Medicare for patients who are taking pills instead of insulin/injections. Will Medicare reimburse it?
2. How much "Glucose testing per day" is required to meet the CGM criteria?
3. If patient is taking insulin 2 times a day, can we bill that claim to Medicare and will it be paid?

Just wondering what you are billing for, the testing kits?  The materials?  Or the testing?
