For marketing purposes.
Name, address, telephone number, email address if available.
As a medical billing company we can solicit business from the Providers, by email/ phone/ direct mail.

Ah, I see. All I can suggest is going onto various insurance websites and looking at their provider directories.

This is a "Google is your friend" situation. Google provider directories in your state, for major insurance companies in your state. Take it from there.

Tallman - these two links are for California. I imagine with a little poking around you can make these links work for whatever State you want the information from.
The first link is free. The second link requires payment (for a database).
