Need help Coding 33418 TMVR

Hi,
I recently started billing and coding for Cardiology again and having a problem getting paid for TMVR cpt 33418. I've billed 33418 modifer 62. I called medicare and they stated I needed to add modifier QO, which I did, but they still denied the claim.  The rep for the product states that patients don't need to be enrolled in the TVT registry any longer. I don't know if that is correct.  If anyone can help me with this, I would greatly appreciate it.

It's hard to help without being able to see the denial codes.  All of the information states that the TMVR is only covered when billed with the 62 and Q0 modifiers.  What is it being denied for?
