Billing Provider

A doctor uses her NPI 1 in box 24j as the rendering provider and uses her business name in box 33 as the billing provider. Instead of obtaining an NPI for her business name she uses the same NPI she reports in box 24j as the NPI for the billing provider. She claims she has billed this way for years and insurance has never questioned it. Can you think of any circumstances which would warrant this method of billing?
And to make matters even worse she uses her name in box 32 as the facility name, same address as the billing provider, and inserts her NPI in box 32a. She has asked me to take over her billing and continue using this same method. Should I be reluctant to take on this client? Is there something I am missing here?
