New Patient vs Est. Patient

Hello,
my provider is a wound care provider. If he sees a patient in an assisted living then the patient gets released and goes home or elsewhere and my provider sees the patient at the new location will the patient be considered as new or est.?

The short answer: The patient is established

The long answer: Determining if a patient is new established is not determined by the patient changing locations. It is determined by other factors, such as if the provider has seen the patient face to face in the last 3 years, or in the case of a group practice, if the providers share the same taxonomy code/specialty.
