Outpatient Billing - Pre, Post and Path charges

Should a patient be billed for all pre, post, and Path charges, including procedural charges on one global bill, or can the charges be broken up and billed separately?  The PT has a copay of $300 for outpt surgery - should that be the only fee, or could fees for pre, post, and path be charged at the normal copay of $50 for each service?

The question is a little vague so I'm not sure if I"m answering it correctly.  I believe you are stating that the patient has a $300 copay for surgery and only a $50 copay for other services.  THe question I believe is, if the surgery is billed as pre, post and path separately will it be 3 $50 copay or 1 $300 copay.  Again, not much info to go by, what insurance carrier?  etc.  But I would guess that the answer is they would still apply the $300 surgery copay.  THe services are still surgical even if broken out into 3 separate charges.
