Who provides the collection agency, the biller or provider?

It depends. The billing company I work for has their own collection agency they use. Sometimes the provider has one they use.

Exactly.  We have one we use and recommend, but ultimately the provider is the one who is the customer of the collection agency.  If they have one they want us to use, we use them.  If not, we recommend ours.
