If a patient comes for routine foot care and it consist of both nail debridement and cutting of the callus every 2-3 months am I able to use modifier 25?

Without seeing the tx note, my answer would be no. The patient needs to be treated for conditions beyond the nails and callouses to have an E/M billed at the same time. Every procedure code has an E/M allowance built into it. So without seeing the tx note, there is no way to say if the E/M is separately allowable.
