Hello,
You're probably all getting tired of all my questions.  I promise once I get this puzzle solved, you won't be seeing quite as much of me!
I work in a community mental health facility and am trying to get our UB-04 complete and billed to Medicare Part A (due to our PHP level).
I understand that Medicare doesn't recognize the CPT code we use to bill Medicaid (H0035).  I think G0129 would be the appropriate HCPCS code.
Can anyone tell me if this is correct?  Also, would 0912 or 0913 be the correct revenue code?

I don't think that G0219 is the right code, that is for Pet imaging, whole body.  The H0035 is for D&A treatment.  912 & 913 do look correct.

I may leave the HCPS code part blank since it's my understanding that it's optional.
Thank you for all your help.  If I ever get a PHP claim to go through to Medicare and get paid, I'll be celebrating!!
