IS anyone out there using this?  99421 - 3 is for when a patient asks you a question thru the portal and the provider answers.  It is NOT telehealth and does not use POS 02 or MOD 95 (per CodingIntel)  I have had no problems except for BCBS.  They keep denying with this: 

OUR POLICY DOES NOT ALLOW PAYMENT FOR THIS SERVICE WHEN PERFORMED IN THE REPORTED LOCATION.

I am using POS 11.   BCBS won't tell me what they want me to use, because "it's not their place to tell me how to bill".
