Consult code denial for number of unit exceeds billable maximum unit

Insurance companies are denying initial evaluation codes for above reason. Is anyone else facing same problem ?
How do we re-bill the claim. Follow-up code is not the appropriate code and pays lot less. When actual time spent is much more than what follow-up code actually pays for.
