cpt 11042 To Medicare

I keep getting denied when billing 11042 bilateral using mod 50 to Medicare.  I have also tried billing separate claim lines for RT and LT.
Does anyone know if Medicare will pay for more than 1 unit of 11042 per day or is there another modifier I should use? 
Having same problem with other insurance :(
