MODIFIERS

Elizabeth:
Good morning, a physician performed a laparoscopic assisted vaginal hysterectomy, bilateral salpingo-oophorectomy, anterior colporrhaphy and bladder sling transobturator tape all in one session.

I coded with the following CPT codes and modifiers 58552-59, 57288-51, 57240-51.

LaCare does not pay CPT code 58552 because it says the modifier is incorrect or missing, but I cannot identify what is incorrect.

Could you please help me confirm which modifiers are appropriate for these types of surgeries, I am new to this topic, I would greatly appreciate your help.
