Does anyone know which date to use for DOS on the "Home Health Certification And Plan Of Care"? (CMS-485 form)

The form has:

Box 2.   Start of Care Date
Box 3.   Certification Periods   from:        thru:
Box 12.  ICD-9-CM w/Date
Box 13.  ICD-9-CM w/Multiple Procedure Codes with dates listed after each   
            code.

Which date or dates are used when filing a claim?

PMRNC:
The start of care (SOC) dates in box 2  is the first Medicare billable visit if
individual is dually eligible for Medicare/Medicaid. If individual is eligible for Medicaid only, SOC
is the first Medicaid billable visit.

The start date in box 2 of the certification period must match the start of care date in box 2
