With Med-Pay do insurance carriers have a time-frame it must remit payment once they have received the bill/claim before any penalties are occurred?

It will vary by state, but the verbiage is not often "pay" the verbiage usually says a carrier has XX days to
"consider"
or
"acknowledge"
a claim.
