UB92 vs UB04

Hi... I'm new to the medical billing world and we will just now begin to process claims for the operating room. I was under the impression that we needed a UB 92 Form but I've been doing some research and it looks like it has been replaced by the UB 04 Form. Which one should I use? I work for a surgeon who operates off his own facility.

You want to use a UB04.  The UB92's are outdated and will be denied.  You may be interested in our UB04 book, too.  You can read about it here http://www.medicalbillinglive.com/UB04-forms-book.shtml.
