Locum Tenen billing on UB 04

Looking for info on what doctor information goes into Blk 76 (UB 04) for the doctor NPI.  I've contacted some of our payers who have said it doesn't matter if it is the Locum's name and NPI since this is a facility claim and is billed with the facility TID/NPI.  I've been told by others to use the Locum's name and our permanent doctor's NPI.  That seems totally wrong to me.  Any ideas?  Thanks!

The name must match the NPI, you can't use one name and another's NPI.  I would put the name & NPI of the attending physician in box 76, whether it was a locum or not.
