95115 allergy shots

How do you code for mutiple allergy injections? Patient supplies own medication & we administer. Patient needs 5 each week. Do I submit 95115 for 1st then 95117 with 4 units for others? Also MD is administering(we don't have a nurse) so I can bill for the time(99211) or just the injections?
