I am not familiar with Florida No-Fault specifically but it sounds like they are finding a way out of paying claims. I would call the company and ask what exactly on the form is not completed properly. If they can't spell it out exactly what they think is not completed properly, then they should pay the claim. I know here in NY you can file a complaint with the state insurance commissioner.
