National qualifiers for Coins and Ded

Is anyone aware of a national qualifier for coinsurance and deductible.  I am aware of the value codes billed on the UB04, but I'm looking for codes to bill on the CMS 1500.

National qualifiers aren't used on the CMS forms.  What are you looking to bill for?

looking to bill for medicare coinsurance and deductible on the CMS 1500

Michele:
So you are billing secondaries out after Medicare processed? 

You would bill the charges that were billed to Medicare with the same amounts as Medicare was billed, but attach the Medicare eob to show how Medicare processed (applied to deductible, or patient responsibility).
