HMB:
I am filing 93701 and the only insurance that seems to paying is Medicare. The rest are denying for office which consists of several patients. I'm using 425.1 as my dx and medicare is paying. Any suggestions.

It is odd that Medicare is paying, but others aren't.  I would start by contacting one of the larger carriers that is denying and get an explanation for the denial.  That should help you determine why they are not allowing it and it may clear it up for all other carriers as well.
