What is the Type of Bill Code for billing Medicare for Outpatient Physical Therapy in a free standing clinic in Michigan on a UB04.

I would try 732, 733 and 734 depending on which claim:

732 - Initial claim
733 - Intermediate claim
734 - Last visit

You may want to call one of your major insurance carriers to make sure that they accept those for type of bill to be sure.
