Anyone out there have a letter they are sending to an insurance company to appeal bundled codes. My letter isn't working.

Questions to help:
1) Is the provider contracted with the insurance company
2) What codes are involved?
3) Did you use any modifiers and if so with which code(s)?
4) Is the benefit plan under ERISA jurisdiction?
