Michele,
I have one more question: if secondary (HMSA HMO) pay $0 due to primary (HMSA PPO) paid in full, but balance (subcriber owes) is still $15 (as per EOB of primary). In this case, should we bill patient $15?

Yes, you need to record that the secondary processed the claim but didn't make payment, and then bill the patient.
