cpt code for coumadin management

Is there a cpt code that medicare will pay for the management of coumadin therapy?  Patient not being seen in office just telephone contact/management. 99363 &99364  not on Medicare fee schedule.

At this time there is not a specific CPT code for coumadin therapy that is covered by Medicare or most major insurers.  The 99363 & 99364 are not covered yet, but there are groups out there advocating to get them covered.
