Locum Tenens Billing on a UB04

We are a FQHC and bill our office visits to Medicare Part A on a UB04.  Can anyone tell me what modifiers to use on a UB04 (CMS1500 equivalent modifier is a Q5 / Q6) to designate locum Tenen billing?

I don't believe the modifiers change when billing on a UB04 as opposed to a CMS1500 form.  I'm not aware of any different one.

Q5 and Q6 are not valid on a UB04.  Do I not have to report a locum tenen to medicare if we're billing on a UB04 (becuase we're an FQHC) even though it is a office visit?

I am not sure.  I tried to research it but the only thing I came up with was the Q5 & Q6.  It didn't indicate whether you had to indicate locum tenen on the UB04.
