letting the ins companies know i will be filing claims

what forms do i use to let the insurance companies- aetna, cigna, medicare,BCBS etc know that i will be filing claims from my home office. i will be using availty and office ally as the clearinghouses and have my forms filled out and ready to go once i have signed my first provider. i just don't know how to inform the insurance companies that i will be submitting claims.

You only need to advise Medicare that you will be the claims submitter, all of your clients will be setup with your clearinghouse under your account. With Medicare you have to submit a change in submitter request using form 855 (depending on the practice)

Do you have to notify medicare everytime you get a new provider or is one form enough? Where do you find this form? Thanks :)

Are you talking about notifying Medicare that you will be billing for the provider? If you are submitting electronically, you will need to do paperwork each time to get 'permission' to submit claims electronically on behalf of the patient. EDI forms are available on the Medicare carriers website. But you also are supposed to notify Medicare that you are the billing agent by completing the appropriate cms form available on the cms website. http://www.cms.gov/CMSForms/CMSForms/list.asp
