billing help- termination of an abnormal fetus

Any advice on how to bill this?

Termination of an abnormal fetus:
Laminaria inserted in the office.  Then the doctor met the patient at the hospital and ruptured the membranes.  She was not present for the delivery of the fetus. 

Pt. has Medicaid

Lots of billing info for this type of situation.  However, Medicaid varies greatly from state to state.  Not sure what state you are in, but the guidelines may be different.
