When you use rev code 0128 do you need to use a CPT code as well. And would the fee be the room and board for the whole inpatient time?

Hi,
My info. is from years of paying facility claims on the insurance side.
This code does not require a CPT - Usually it is seen with discription written on UB04. The R&B fee is per diem.
