No Fault Insurance claim

Hi Michele, or anyone

Could you advise me on how to file a no fault insurance claim for physical therapy? what procedure to take, what information do I need from the patient. What forms do I use? What code do I use?

Generally, Now Fault Insruance claims donot require any special instructions for Physical theraphy alone.

Use the general billing guidelines as we do for general billing at case of No Fault insurance.

Pick up a copy of FIR if he/she could give us, standard disclosure form & details about the accident to print accident date on HCFA

Below are the general Physical theraphy codes

CPT 97001 & 97002 are for Physical Theraphy Evaluation and re-evaluation

CPT 97010 to 97028 is for Physical theraphy treatment modalities

CPT 97032 to 97039 is for Physical theraphy treatment modalities
