what is entity code?

hello, currently billing for cpo and on testing phase, recieved a message claim has not been paid due to mission entity code? what does this mean? and where would it be located on 1500 claim form?

Need more info.  I don't know what cpo and on testing phase means, or what is a mission entity code?

missing entity code, billing physician care plan oversight, for home health

Ultraserve:
Call the insurance company that you billed and asked them what exactly the need and what entity code the need and where you should place it, if it's a paper claim, if electronic claim, click on the electronic reject field and it will give you all the options and description, depending on what software you use.
