Emergency Room Charges

I am writing a sort of "how to" guide for our ER nurses regarding their ER charges i.e. IV Infusion Substance, IV Push, IV Hydration etc... (CPT 96365, 96374, 96360 et al).  Any ideas about where I can get detailed explainations of these charges and others.  The CPT book is very basic and doesn't explain in detail.
