Medicare as a secondary payor.

I have been having difficulty with Medicare claims that are secondary to a primary insurance.  Do you have any advice or links you could direct me to that would help me with filing secondary claims to Medicare.  An example of the CMS 1500 form would be great.  If you could help in any way, it would be greatly appreciated.
