I am applying for a CLIA# for my provider(Family Practice in the process of an Urgent Care contract) should I apply for a Waiver or PPM? and if so, will this change once he opens his Urgent Care? Thank You again.

If you are applying for a CLIA# I'm not sure why you would need the waiver. I tried to look into it but didn't come up with anything. If you are applying for the CLIA# under the provider's name and tax ID# then it shouldn't change when he opens the urgent care.
