OP Diabetes Self-Management Training: Medicare Billing on UB92 claim

If the DSME is furnished outside of the hospital, if this noted anywhere on the UB92 claim form when billing Medicare?

UB92's are no longer used, it's a UB04.  Not sure what you mean by DSME.  Who is billing for it?
