After insurance has payed their part and you have paid your part of a bill... what happens to the remaining balance?

DMK:
If the doctor is a contracted provider, the balance is written off.  Your Explanation of Benefits will show it as "Not Allowed" or something to that effect.  The big thing to realize is that the doctor can charge whatever he/she wants, but they don't get it.
