Code 36556 to Medicare

Are there certain specifics to getting code 36556 paid by Upstate Medicare.  I am billing in NY.  Medicare is denying my claims saying service is not paid seperately.  I am also billing codes 93503 and 36620 as well as the primary procedure of 00562.  I am billing for anesthesia.  Thanks for all your help.
