does anyone know the mailing address to mail form MEDICARE cms-588 EFT authorization aggrement.  I've look on the website but cannot find a mailing address.. Thanks

It depends what state you are in.

You need to go to the web address for the Medicare carrier for your area.  Go to the contacts/addresses and mail it to the provider enrollment address.  Almost all have it posted on their web site.  If you can't find it, call the provider contact number and ask.  I find it much easier to locate the address online than to make the call.
