REFERRING PHYSICIANS

I AM BILLING FOR A FACILITY, BUT NEED TO TRACK WHO THE PERFORMING SURGEON IS, CAN I USE THAT PROVIDER INSTEAD OF THE REFERRING PRIMARY PHYSICIAN.  THANKS IN ADVANCE!

What do you mean you have to track the performing surgeon?  The UB04 has a place for attending physician and operating physician.  You should put whoever is overseeing the patients care in the attending physician.

what type of facility??? ASC(freestanding) Hospital, Hospital ASC??? I know NOTHING about UB04 but if it is for the facility of a freestanding ASC I could prob answer. Would need more infor so as not to steer you in the wrong direction.
